Chronic Disease Self Management Program

The Living Healthy With a Chronic Condition Program is comprised of evidenced based programs to  help you gain control over your health and improve your quality of life. The programs are available for anyone over the age of 18 living in Onondaga County. Each participant receives a resource booklet,  as well as the CD used during the workshops.

The programs are a six session, peer-led health education program for people and/or their caregivers with any type of ongoing health condition and complements the care of the healthcare that person may already be receiving.

 

There are three self-management programs to choose from:

  1. Chronic Disease Self-Management Program for participants and/or caregivers with any type of chronic condition(s).
  2. Peer Support for Adults with Type 2 Diabetes for participants and/or caregivers who are pre-diabetic or diagnosed with Type 2 Diabetes.
  3. Chronic Pain Self- Management Program for participants and/or caregivers who are managing chronic pain.

  • Making weekly action plans, 分享经验, and helping each other solve problems they encounter in creating and carrying out their self-management program
  • Decision-making and problem-solving skills
  • Techniques to deal with emotional problems such as frustration, stress, fatigue, isolation
  • Fatigue and sleep management

  • Developing an appropriate exercise plan for maintaining and improving strength
  • Appropriate use of medications
  • Communicating effectively with family, friends and health professionals
  • Nutrition and healthy eating
  • How to evaluate and make informed treatment decisions
  • Planning for future health care
  • Peer Support for Adults with Type 2 Diabetes Program will look at techniques to deal with the symptoms of diabetes, 超/低血糖

Tai Chi For Arthritis Programs are 也提供了. These workshops are designed to reduce the fear of falling and increase activity levels among older adults.  These programs enable participants to achieve significant goals. They gain confidence by learning to exercise to increase strength and balance, set goals to increase activity, view falls as controllable and make changes to reduce the risk of falls.
